การรับรู้และประสบการณ์ของผู้ป่วยเบาหวานชนิดที่ 2 ที่เข้ารับบริการทันตกรรมในโรงพยาบาลกงหรา จังหวัดพัทลุง

Abstract

การวิจัยในครั้งนี้เป็นการวิจัยแบบภาคตัดขวางเชิงสำรวจร่วมกับการวิจัยเชิงคุณภาพ มีวัตถุประสงค์เพื่อศึกษาการรับรู้และประสบการณ์การเข้ารับบริการทันตกรรมของผู้ป่วยเบาหวานชนิดที่ 2 และศึกษาระบบบริการทันตกรรมสำหรับผู้ป่วยเบาหวานชนิดที่ 2 ของโรงพยาบาลกงหรา จังหวัดพัทลุง การวิจัยแบบภาคตัดขวางเชิงสำรวจเลือกกลุ่มตัวอย่างในการศึกษาโดยการสุ่มตัวอย่างแบบบังเอิญจากผู้ป่วยเบาหวานชนิดที่ 2 ที่ลงทะเบียนไว้กับกลุ่มงานเวชปฏิบัติครอบครัวและชุมชน จำนวน 110 คน เก็บข้อมูลโดยการสัมภาษณ์ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน กลุ่มตัวอย่างในการวิจัยเชิงคุณภาพใช้วิธีการสุ่มตัวอย่างแบบเฉพาะเจาะจง ได้แก่ ผู้ป่วยเบาหวานชนิดที่ 2 จำนวน 10 คน ทีมสหวิชาชีพจำนวน 11 คน เก็บรวบรวมข้อมูลใช้เทคนิคการสัมภาษณ์แบบเจาะลึกและนำบทสัมภาษณ์ไปถอดข้อความจากเทปแบบคำต่อคำและตรวจสอบความถูกต้องของข้อมูลด้วยเทคนิคสามเส้า วิเคราะห์ข้อมูลโดยใช้การวิเคราะห์เนื้อหา

ผลการศึกษาพบว่า กลุ่มตัวอย่างรับรู้ว่าต้องตรวจสุขภาพช่องปากและรับทราบผลการตรวจร้อยละ 97.3 และเข้ารับการรักษาทางทันตกรรมที่โรงพยาบาลกงหราในรอบ 1 ปีที่ผ่านมาร้อยละ 13.6 การไม่รับรู้การมีความเจ็บปวดหรือไม่สบายจากโรคในช่องปากในปัจจุบันพบร้อยละ 90.2 รับรู้ว่าสุขภาพช่องปากโดยรวมของตนเองอยู่ในระดับแย่-พอใช้ร้อยละ 72.3 ข้อมูลที่ได้จากการสัมภาษณ์เจาะลึกแสดงให้เห็นมุมมองของผู้ป่วยเบาหวานชนิดที่ 2 ว่าระบบบริการทันตกรรมไม่อำนวยความสะดวกต่อผู้รับบริการ ผู้ป่วยขาดความตระหนักเรื่องสุขภาพในช่องปากและมีความเข้าใจไม่ถูกต้องในการรักษาโรคปริทันต์ ในมุมมองของทีมสหวิชาชีพ พบว่า ผู้รับบริการขาดการเข้าถึงข้อมูลบริการ ตัวชี้วัดบริการไม่เหมาะสมกับผู้ป่วยเบาหวานชนิดที่ 2 และการวางแผนบุคลากร งบประมาณและการบริหารจัดการยังไม่เหมาะสม สรุปผลการศึกษาได้ว่า ผู้ป่วยเบาหวานชนิดที่ 2 มีการรับรู้เรื่องสุขภาพในช่องปากว่าพวกเขามีสุขภาพช่องปากในระดับแย่ถึงระดับปานกลาง แต่ยังละเลยในการมารับบริการทันตกรรม เนื่องจากระบบบริการทันตกรรมยังไม่เหมาะสมยังผลให้ผู้ป่วยไม่สามารถเข้าถึงบริการได้

This research was a cross-sectional survey with qualitative research. Objectives were to study perception and experience of dental services utilization in type 2 diabetic patients and to describe the dental services for type 2 diabetic patients in Kongra Hospital, Phatthalung. One hundred and ten of type 2 diabetic patients from Department of Family Clinical Practice and Community were attended in accidentally sampling. All subjects were completed the questionnaire. The statistic use for data analysis included frequency, percentage, mean and standard deviations. In addition, purposive sampling was used to select 10 patients with type 2 diabetes mellitus, and 11 staffs of the multidisciplinary team. In-depth interview technique was used to be an instrument for all participants and transcription data using word by word for a content analysis.

Results of the study shown that in the year of study, the perceived dental checkup rate was 97.3 % while the dental service utilization in Kongra Hospital was 13.6%. Their perception on no pain and no discomfort from dental diseases was 90.2% and their perception of dental health was poor to moderate levels. Data support from in-depth interview shown type 2 diabetic patients faced to inconvenient service system, unawareness of dental problems, and misunderstanding of periodontal disease. In the same way, a multidisciplinary team view supported that these patients lack of health information accessing, inappropriate indicators, and inappropriate planning on manpower, budget, and management. In conclusion, type 2 diabetic patients perceived that they were in the poor to moderate level of dental health. However, they ignored dental services utilization. The reason was service system due to difficult accessibility.
